Request Your Medical Records

The Chartlocker platform allows you to submit your HITECH Act Request (a fancy law passed by Congress that allows you to personally request your electronic medical records) for medical records to any provider in the United States. Simply select 'Request records' from your chart to search for your provider and create the request form. After you create, sign and submit your request form, the Chartlocker team will send your HITECH Act Request directly to the identified provider.
